Capricornia Club Release and Indemnity

I/we agree to comply with the rules and by-laws of Capricornia Blokart Club Inc. (CBC) & Australian Blokart Association (ABA).

In consideration for your permitting registration of this child(ren) or yourself to participate in the following blokart activities sponsored by the CBC, the participant(s) does hereby release the CBC and all affiliated and associated organisations, together with their representatives, members or agents, of and from any and all demands of any kind or nature whatsoever, arising out of or in any way related to any participation in such activities, including any personal injury or death which may be suffered or incurred as a result of participation in such activities whether or not caused by the negligence or wrongful acts of such persons or any agents or members of the CBC and I do further agree to indemnify and hold harmless each of them of and from any and all claim, demands or action of any kind or nature whatsoever arising out of any injury or demands incurred by the registered participant.

I/we consent to the collection of the above details by CBC & ABA for the purpose of membership records, sponsors and other funding agencies, the news media or other organisations for the development of Blokart sailing and for the benefit of CBC & ABA members.

Privacy
The CBC & ABA requires the information on this membership form for its own records, administration and statistical purposes. Sponsors, funding agencies and other relevant organisations may require non-identifying statistical information. Existing relevant organisations are:

• IBRA – International Blokart Racing Association
• Affiliated Australian Blokart clubs.

Please note the CBC and ABA will not sell or pass on member information to anyone else.
